PAY MY DOCTOR: WHERE'S MY CLIENT ID?

Alright, so I need to settle my invoice with Dr. Smith. I went to their website and it's asking for a patient ID number. Problem is, I have absolutely no idea where that can be found. I've looked my emails, appointment confirmations, but nothing! Could someone please guide me? Is there a phone number I can reach out to? Maybe on my last bill?
